Please keep in mind that if you directly disclose personally identifiable information or personally sensitive data through public areas of this website or other public channels, this information may be collected and used by others.

The Medical Partners Group encourages you to review the privacy statements of websites you choose to link to from The Medical Partners Group website so that you can understand how those websites collect, use and share your information. The Medical Partners Group may also use your personally identifiable information to inform you of other products or services available from The Medical Partners Group and its affiliates, and may contact you via surveys to conduct research about your opinion of current services or of potential new services that may be offered.

The Medical Partners Group does not sell, rent or lease its patient or customer lists to third parties. The Medical Partners Group may, from time to time, contact you on behalf of external business partners about a particular offering that may be of interest to you. In those cases, your unique personally identifiable information (e-mail, name, address, telephone number) is not transferred to the third party. In addition, The Medical Partners Group may share data with trusted partners to help us perform statistical analysis, send you email or postal mail, provide customer support, or arrange for deliveries. All such third parties are prohibited from using your personal information except to provide these services to The Medical Partners Group, and they are required to maintain the confidentiality of your information.

Cookies

Cookies and tracking

A cookie is a small text file that is placed on your device by a web page server. Cookies cannot be used to run programs or deliver viruses to your computer, and a cookie can only be read by a web server in the domain that issued it to you.

This website does not currently use cookies for analytics, advertising or tracking, and it does not require cookies to display practice information or to use the browser-only patient forms.

Most web browsers let you review, block or delete cookies through your browser settings. If the practice later introduces website features that rely on cookies, this statement will be updated to describe them and the choices available to you.

Protected health information

HIPAA and your health information

As a healthcare provider, The Medical Partners Group is a covered entity under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Protected health information that we collect in the course of providing care, such as your medical records, treatment information, and billing records, is used and disclosed in accordance with HIPAA, its implementing regulations, and applicable state law.

This Statement of Privacy governs information collected through this website. Our handling of your protected health information is governed by our Notice of Privacy Practices, which describes how your health information may be used and disclosed and how you can exercise your rights, including requesting access to your records, requesting corrections, and receiving an accounting of disclosures. A copy of the Notice of Privacy Practices is available at our office and on request.

Please do not send medical information by standard email or text message; these are not secure channels. For clinical questions, or to exercise your rights regarding your health information, call the practice at 559-221-7500 or contact our office in writing.

Retention and deletion

How long information is kept

Data retention

The Medical Partners Group retains your personal information only for as long as it is necessary to provide you with our services and for the purposes described in this statement, or as required to comply with our legal, regulatory, tax, accounting, and reporting obligations. Because we are a healthcare provider, certain medical and billing records are subject to mandatory retention periods under applicable federal and state laws (for example, HIPAA and state medical record retention requirements), and we will retain those records for the minimum period required by law. When your information is no longer needed and is no longer subject to a legal retention obligation, we will securely delete, destroy, or de-identify it.

How to request deletion of your data

You have the right to request that we delete the personal information we hold about you. To request deletion of your personal data, email us at info@ncaremd.com with the subject line “Data Deletion Request”, or call the practice.

When we receive your request, we will verify your identity and delete your personal information within 30 days, except for information we are legally required to retain (such as medical and billing records subject to mandatory retention periods). Once any required retention period expires, that information will also be securely deleted or de-identified. We will confirm in writing once your request has been completed.
